If you have acne, you’re among more than 70 million people in the United States who have suffered from this skin condition at some time in their lives. It is so common that acne affects about 80 percent of Americans 20 to 30 years old. During the teenage years, acne is more common in boys than in girls, but in adults it’s more common in women.

Despite the fact that it’s so commonplace, there are many misconceptions about acne, says Guy Webster, MD, PhD, a clinical professor of dermatology at Jefferson Medical College in Philadelphia and founder of the American Acne and Rosacea Society.

Getting to the Root of Acne

Whether you call it acne, pimples, or zits, in order to treat the condition, it’s important to understand the causes:

• Clogged pores and bacteria: In your teens, the glands in the skin begin secreting sebum, an oily substance. This normally comes out through the pores, but in some people, sebum clogs up in the pores, allowing a bacterium, called P. acnes, to begin to grow.
• Hormones: In your teen years, hormones start changing and affecting your body, including causing acne. This also happens during pregnancy, which explains why pregnant women or women having their periods often have acne breakouts. Hormones released during stressful times can also cause acne.
• Genetics: You may be more likely to develop acne if your parents had acne when they were younger.

The Right Acne Treatment

There are many ways to take care of acne, depending on what causes it and how bad it is. Moderate and severe acne usually needs acne treatment recommended by a doctor, but mild acne, blackheads, whiteheads, and a few pimples can usually be treated at home.

Dr. Webster says one big misconception is that acne is caused by dirty skin. “The goal is not to scrub acne away,” he says. “If you scrub, you’re taking off skin, and there’s a reason for the skin being there.” Skin is a protective barrier.

Here are some tips that Webster shares with people who have acne:

• Wash gently; don’t scrub.
• Use a gentle soap to wash your face.
• Wash with your hands, not a washcloth or “scrubby.”
• Use a 5 percent benzoyl peroxide product.
• Treat your whole face — don’t “spot treat.” This way, you’re treating pimples still under the skin but not yet visible.

Other tips to keep acne from getting worse:

• If you’re a male, be careful shaving.
• Don’t pick or scratch at pimples.
• Avoid the sun. While many people feel that sun exposure makes their acne better, this is not always so. The rays can also cause other unwanted issues, such as premature aging and skin cancer.

When Should I See a Doctor for Acne Treatment?

According to Webster, if the pimples are leaving scars or if your treatment isn’t working, then it’s time to see a family doctor or dermatologist.

When the sensitive skin on the bottom of the feet and heels becomes too dry, it can split open, leaving painful cracks called fissures on your heels. Those cracks may not only make it painful to walk, but can also lead to serious infections.

Cracked Heels: How They Happen

Cracks in the heels are generally caused by insufficient moisture. These cracks can become sore and may even bleed. Seriously dry feet can occur for a number of reasons, including:

• Cold winter weather
• Dehydration, or not drinking enough water
• Not moisturizing your feet
• Taking very hot baths or showers
• Soaking in a hot bath for too long or too frequently
• Using harsh, drying soaps on your feet
• Scrubbing feet dry
• Having diabetes

“Heel fissures and cracks occur when the skin loses its moisture content and dries out and cracks,” says Alan K. Mauser, DPM, a podiatrist in Louisville, Ky.

When the feet become too dry, heel fissures can develop quite easily. “It’s kind of like if you put plaster on a balloon and let it harden and blow up the balloon, the balloon will expand the plaster and crack. The heel pad wants to expand outward, but the skin is not pliable enough to expand with it, so it cracks,” Dr. Mauser explains.

As the skin cracks, it may begin to bleed. These deep heel fissures can allow bacteria and viruses to enter the body, leading to infection and illness.

Cracked Heels: Risk Factors

The two biggest risk factors for cracked heels are diabetes and obesity, notes Mauser. Diabetics are likely to experience cracked heels because damage to nerves in the feet from uncontrolled blood sugars can cause dry skin. People with diabetes are even more likely to sustain an infection from cracked heels than non-diabetics. If you are diabetic, it is important to examine your feet frequently for signs of cracks or infection.

Obesity increases your chances of having cracked heels because there is even more weight on the heel pad, which causes it to expand out further. Dry skin is unable to handle the added pressure and cracks.

During the winter months, however, anyone can have dry, cracked heels. Regularly taking long, very hot baths and showers can also exacerbate this condition. Additionally, people who don’t regularly moisturize their feet with a good, oil-based lotion or moisturizer are more likely to experience heel fissures. Not drinking enough water and poor nutrition are also risk factors for cracked heels.

Diagnosing Cracked Heels

If your heels are cracked and painful, and the condition doesn’t improve with an over-the-counter foot lotion, visit a podiatrist to treat the problem. If your heel fissures are “severe enough, we’ll make suggestions and help you through it,” Mauser says. A podiatrist can treat any associated infection and also offer solutions to help your skin heal properly.

Mention the word “Botox” at a party, and you are certain to get all kinds of reactions.

The truth is that most people do not know all the facts about Botox. In the world of cosmetic enhancement, Botox is used to eliminate frown lines, crow’s feet, and other wrinkles.

Botox injections are one of the most common cosmetic procedures performed today. The Food and Drug Administration has approved use of the drug — botulinum toxin type A injection — to temporarily remove frown lines, but it also has been approved to relieve uncontrollable tightening of neck muscles (cervical dystonia) and eyelid muscles (blepharospasm), to ease crossed eyes ( strabismus), and to control severe underarm sweating. Other uses would be considered “off-label” and potentially dangerous.

Ask most people who have had Botox injections how they like it, and many are ecstatic. Mary Tator, a registered nurse in Baltimore, for instance, has had regular Botox for years and says she “can’t live” without her twice-a-year injections.

But Botox may not be right for everyone. And, most importantly, the anti-aging results will depend on the skill of the practitioner injecting the drug.

The ABCs of Botox

Botox is a purified neurotoxin made by a particular kind of bacteria, clostridium botulinum. A neurotoxin is a toxic substance that normally damages or destroys neurons, the main building blocks of your nervous system. If you were to ingest the bacteria, you’d get botulism, a dangerous disease of the nervous system. However, when a tiny amount of the neurotoxin, not the bacteria, is skillfully injected in the right spot, anti-aging results can happen: Facial wrinkles may disappear.

According to Scott Gerrish, MD, a nonsurgical skin care specialist with offices in Virginia and Maryland, Botox temporarily inhibits neurons from “talking” with muscles wherever it is injected. “Because the muscle is unable to move under the skin, which is what causes skin creases, frown lines are eliminated for up to three to six months, and a simple retreatment is all that is required to maintain a smooth brow,” Dr. Gerrish says.

Pros:

·         Botox injections are a quick and simple procedure done in your doctor’s office in minutes.

·         There’s no downtime from work or other activities.

·         Most people who have had Botox injections are very satisfied and expect to return for re-treatment.

Cons:

·         Results are temporary.

·         Possible side effects include bruising or bleeding at the injection site.

·         Costs vary but range from $200 to $500 per facial area. The amount charged for treatment may vary depending on your doctor’s experience and the part of the country you live in.

·         There have been deaths linked to Botox injections, but they were not from cosmetic use. The deaths involved cases in which children were injected with Botox for muscle spasms, and the toxin spread beyond the injection site. The FDA concluded in 2008 that Botox is safe and that adverse reactions were “related to overdosing.”

·         Some firms and doctors have been found guilty and imprisoned for selling or using research-grade botulinum toxin, causing harm to unknowing patients.

Botox: Safety First

Gerrish cautions patients not to take any ibuprofen (Advil or Motrin, for example) or aspirin for a week before having a Botox treatment to minimize bruising and bleeding. Other precautions to take:

If your skin is oily, you may be bothered by the shine, greasy texture, and breakouts. But don’t blame the foods you’re eating. “There is no data to show that you will produce more oil if you consume things that are more oil-based,” says Rebecca Kazin, MD, an assistant professor of dermatology at Johns Hopkins University and medical director of the Johns Hopkins Cosmetic Center in Baltimore. “The fact is that people who have oily skin were probably born that way. There is not much they did to get it and there is not much they can do to prevent it.”

Oily Skin Care Dos

The good news is there are several ways to manage oily skin, experts say:

·         Wash with salicylic acid. “Cleansers that contain salicylic acid penetrate into the pores and help remove fats that clog the pores and lead to blackheads,” says Leslie Baumann, MD, director of the University of Miami’s Cosmetic Medicine and Research Institute.

·         Use a retinoid at night. Whether over-the-counter or prescription, retinoid products decrease oil production in the skin. “This helps reduce blackheads and may lower sebum production,” says Dr. Baumann.

·         Use oil-free foundations. To avoid creating more of a shine and potentially clogging pores, make sure your foundation is oil-free. Use a powder blush instead of a cream formula for the same reasons.

·         Use blotting paper. Washing your face during the day can be difficult, especially for women who wear makeup. Instead, dermatologists recommend blotting paper. “You can absorb the extra oil without washing your face, and they are not irritating to the skin,” says Dr. Kazin. Paper towels can be substituted in a pinch, but blotting paper is better because it contains a small amount of powder, which evens out skin color.

Oily Skin Care Don’ts

Knowing what not to do can be just as important as knowing how to properly care for your oily skin. Experts weigh in on what to avoid:

·         Don’t use creamy or milk cleansers. “These types of cleansers deposit unnecessary lipids oils on the skin, which can make you feel even oilier,” says Baumann. Better to stick with salicylic acid or glycolic cleansers, or gentle liquid cleansers. We recommend Dead Sea Premier Facial Gel.

But Baumann warns: “They do not have enough SPF, even if it says so on the label. To get the SPF stated on the label, you’d need to use 15 times the amount of powder you would normally use.”

·         Don’t overwash. Oily skin isn’t a hygiene problem, so extra cleansing isn’t the answer. “If you wash too much, you can strip your face of the essential oils that serve as a barrier to a lot of irritants,” Kazin says. “This can cause your face to become red and raw. It’s better to wash twice a day and use blotting paper when you feel shiny throughout the day.”

As far as protective wrappings go, skin has everything else beat. Your skin is your body’s barrier, protecting you against the elements, not to mention disease. You can return the favor with just a few simple skin care basics, and by getting attention from a dermatologist when your skin experiences any condition that’s out of the ordinary.

Skin Care Basics: It’s Elementary

Cleanse, nourish, protect — those are the three skin care basics that will keep skin looking, feeling, and functioning great. Because these skin care steps all rely on products, start by choosing them wisely. Remember that skin care products aren’t regulated by the U.S. Food and Drug Administration (FDA), so you need to be a careful shopper.

Look for products tailored to your skin type, but at the same time, beware of too much hype about the results you can expect. “There’s no correlation between how expensive a product is and how well it works,” says Mathew M. Avram, MD, board-certified dermatologist and director of Massachusetts General Hospital Dermatology Laser and Cosmetic Center in Boston.

Keep in mind that serious skin conditions usually need the care of a dermatologist, a skin specialist, who can prescribe effective treatments that have FDA approval.

Skin Care Basics: Your 3-Step Master Plan

With only a few products and a few minutes, skin care will be a breeze:

1.      Cleanse. Dry and sensitive skin need cleansing just once or twice a day, but even oily and acne prone skin will benefit from gentle skin care — overwashing can be irritating and chap the top layer of skin, and won’t impact the amount of oil you produce. Cleansing is most important at night — yes, just when you may feel the most exhausted — but it’s a must to remove old makeup and the grime of the day. The old-school three product skin care system used to call for a liquid toner between the cleanser and the moisturizer, but this isn’t necessary, says Dr. Avram. Exfoliating can be part of cleansing, but it’s more of a weekly or twice-a-week step to slough off any dead cells to help reveal a rosier complexion.

2.      Nourish. Moisturizer works by sealing in skin’s natural moisture. If you have dry skin, you may need a rich formula by day and a thicker one at night. “For dry skin, the best time to apply moisturizer is right after you shower, when skin has moisture in it,” says Avram. If you have oily skin, you may not need one at all. “Oily skin won’t need a moisturizer to same extent as dry skin, but does need sunscreen,” says Avram. “Look for gel or lotion products instead of creams.”

3.      Protect. In fact, it can’t be said often enough: Sun protection is vital for every skin type, every complexion, and every day because, by far, what causes skin to age and causes skin cancer is the sun, says Avram. He adds that wearing a hat, avoiding peak sun hours, and applying sunscreen is crucial to avoid skin discoloration and wrinkles over time. Look for cosmetics, from moisturizer to your foundation, that include SPF, but if you’re going to be in the sun for any longer than it takes to get in and out of your car, use a sunscreen that offers UVA and UVB protection (at an SPF of at least 15) to guard against sun exposure and sunburns as well as skin cancer. And don’t forget to reapply often.

Your makeup can protect your skin in other ways, too. Today’s formulas often make cosmetics more like skin care products. These beauty workhorses may contain ingredients that can soothe dry skin, lessen wrinkles, or fight pimples, depending on what your skin needs. The American Academy of Dermatology suggests looking for products with antioxidants that increase skin health by fighting cell damage, and reinforce sun protection.

Another way to protect your skin is to quit smoking. “Smoking will cause skin to prematurely age and wrinkle earlier,” says Avram.

Last, but definitely not least, protect from head to toe with a body check for suspicious moles and any other growths. “You should check yourself, or ask your spouse check you, every few months,” says Avram. “Look for changes in color or size or if a mole bleeds.” Also, if a mole suddenly appears or just doesn’t look right to you, see a dermatologist for an evaluation.

Skin Care: When to See a Specialist

You might only think of a dermatologist in terms of treating a serious skin condition, but a skilled dermatologist can advise you on skin care and skin protection as well as any problems with the health of your hair and your nails. Some dermatologists specialize in areas like mature skin and are a great resource for effective anti-aging techniques and products that can be part of your everyday skin care.

Talk to your primary care physician about when to see a dermatologist for a body check and which of these common skin care woes and conditions will benefit from a visit to this specialist:

·         Acne: clogged pores, pimples, and inflammation

·         Brittle nails and other nail problems

·         Contact dermatitis, allergic or irritant skin reactions to certain products

·         Excessive perspiration

·         Hair loss

·         Hives

·         Psoriasis, the skin condition characterized by scaly, inflamed skin plaques

·         Rosacea, the skin condition characterized by broken blood vessels, flushing, and pimples that may be mistaken for acne

·         Sensitive skin: burning and stinging from products without any obvious irritation
